In order to register your child for school at either St. Mary or St. Thomas School, you should bring the following identification documents to the school office:

• Birth Certificate OR Passport
• Social Security Card
• Current Medical/Dental Forms (doctor's physical exam, with required vaccinations)
• Transfer papers (if transferring from another school) 
• Copy of most recent report card and/or achievement test scores (for grades 1-8, if transferring from another school)

If your child is Catholic, you should also bring:

• Baptismal Certificate
• First Communion Certificate (if applicable)
• Confirmation Certificate (if applicable)

The following forms (available in the school office) most be completed and returned:

• Registration Form
• Tuition Agreement (one per family)
• Emergency card (front and back)
• Medical/Emergency Information
• Home Language Survey

Students applying for grades 1-8 will be interviewed prior to acceptance of the application.  Please set up an appointment with the school office.
